Welcome to Diva's Online Booking Form. This form is intended for the purpose of booking Bachelor/Bachelorette or Private Parties for all occasions. All of these fields are required!
When paying with a credit card, you must make sure that you type your name in exactly the way that your name appears on the credit card. You must also make sure that you're address is the same address that the credit card company has.
If you will be paying with a method other than a credit card, your payment/deposit MUST be received before any services are rendered. First come, first serve.

Please read before booking a party:
- Party rates based on 10 or more guests per performer.
- If there are less than 10 guests the rate is $180 per performer, per hour
and rates DO NOT INCLUDE TIPS OR MILEAGE.
- Mileage charge minimum= $10 or $0.60 per mile
- All shows are scheduled for up to the rate you choose at the time
of the booking.
- Diva's bod Squad will not be held responsible for any time delays
due to weather conditions or unperceived events. All shows are guaranteed
for the length of the rate you choose at the time of booking.
- Performers may stay longer, provided they have no other engagements
that night and are being compensated for additional time.
- Deposits are non-refundable but we can reschedule your show if necessary.
- There will be a $20 fee added for any changes made after the initial
booking. Balances are due on arrival and tipping is mandatory. Failure
to pay the balance or tip performers will be considered a breach of
contract, resulting in the performer(s) leaving.
- It is the sole responsibility of you the contractor to verify that
all guests attending the performance are 18 or older. You are also responsible
to monitor your guests behavior.
- Unacceptable behavior will result in the performer(s) leaving early.
- Performers are NOT prostitutes, call girls or escorts; and abide by
all Local and State Laws. Please respect the performers and their
security drivers.
- Absolutely NO video or photo camera's are allowed while the performance
is going on. Please respect the performer(s) and ask permission before
taking photos or video recording.
